You may pre-register with our office by filling out our secure online patient registration form. After you have completed the form, please make sure to press the submit button at the bottom of the page to automatically send us your information.
On your first visit to our office, we will have your completed form available for your signature.
Patient paperwork can be filled out before you arrive in our office. If registration paperwork is completed, please arrive 5 to 10 minutes early.
If you prefer to fill out the paperwork in the office, no problem. Just arrive thirty minutes prior to your appointment to complete the paperwork.
Please bring the following documents for your first visit to our Thornton or Greeley office.
- A list of medications
- Your insurance information
- A panoramic x-ray
If you have an x-ray that was taken within the last year, please ask your dentist/orthodontist to provide it. If not, we will take a digital panoramic x-ray during your initial visit.
