Register Before You Arrive

You may preregister with our office by filling out our secure online patient registration form. After you have completed the form, please make sure to press the submit button at the bottom of the page to automatically send us your information. On your first visit to our office, we will have your completed form available for your signature.

When you arrive for your appointment, please be sure to bring the following:

• A list of medications
• Your insurance information
• A panoramic x-ray:
  If you have an x-ray that was taken within the last year, please ask your dentist/orthodontist to provide it. If not, we will take a digital panoramic x-ray during your initial visit.
