Request An Appointment

During your first appointment, we will learn more about your health history, take and review x-rays, do an oral exam, and discuss your treatment options.

Before you arrive for your first appointment you will be asked to provide:

This includes your contact and insurance information as well as a list of your current medications. You can complete this online, prior to your visit, or please arrive thirty minutes prior to your appointment to complete the paperwork.

If you have an x-ray that was taken within the last year, please ask your dentist/orthodontist to provide. If not, we will take a digital panoramic x-ray during your initial visit.
